Piriformis Syndrome

The human body is a complex machine in which a problem in one area may be felt in another. An impingement of a nerve root, for example, might result in pain anywhere along the length of that nerve. At the Carrollton chiropractic office of Dr. Peter Lazarnick (Dr. Pete), we treat the body holistically, using minimally invasive therapeutic techniques to restore its wellness. This includes putting the body’s biomechanics in the best position to balance themselves, which is why this week we wanted to discuss an often-neglected source of back pain: piriformis syndrome.

The lumbar vertebrae that make up our lower backs have to bear a lot of weight and provide stability to our pelvic region. The sciatic nerves which lead to our legs emerge from the lumbar vertebrae and pass through the dense muscles of our hips on their way down our thighs. Among the muscles they pass through are the wedge-shaped piriformis muscles, which connect the lumbar vertebrae to the tops of the femurs. There is one piriformis muscle on each side of the body, and the sciatic nerve for that side must skirt around it or pierce through it. Either way, if the piriformis muscle becomes swollen, the sciatic nerve may be compressed. This could result in pain, tingling, or numbness between the lower back and the foot, as well as causing soreness in the lower back.

Piriformis Syndrome typically only affects one side of the body, and the patient’s pain tends to be worse when they are seated or walking up stairs. Since problems in the soft tissues won’t typically show up on x-rays, diagnosis is done through physical tests and the process of elimination. Patients are advised on stretches that can loosen a tight piriformis muscle, and provided with anti-inflammatory treatments such as electric muscle stimulation and heat therapy. We may also provide chiropractic adjustments if a tightened piriformis muscle has subluxated the lumbar vertebrae.
